I hereby consent to receive spiritual scientific care/counseling and enroll in the Institutional Review Board (IRB) of this private trust consortium. I recognize that the counselor is a channel for Divine intervention and follows the teachings, research, and methodologies of Spiritual Scientific Care/Counseling™ research.
1.2 I agree to take part in the Spiritual Scientific Care/Counseling research process with knowledge of the above.
1.3 This document and all related data and information are proprietary and intellectually protected. Any unauthorized duplication, removal, modification, dissemination, or distribution is a violation of law.
